Hospital health service organization from the perspective of nursing academics

The nursing while management care instruments are essential to the systematization of assistance in its several aspects, since that the nurse is a master of planning techniques, beyond the technical-scientific knowledge, provides improvements in care practices in the health context5. The care production process, with its progressive complexity, in turn requires the organizations of the managements reorganizations to answer the demand for client service in the Health Unic System. In this context, in detriment of population demand association to the health work process, it is necessary to analyze the factors affecting the care planning1. The nurse is the team leader and must afford conditions and artifices so that care occurs safely and with quality. Ensuring such care can occur through management actions, making possible to reach satisfactory goals across the junction between mission, vision, values, institution’s strategy, in the search for maximizing the care quality4,2.. The incitement and the participation of all professionals in the decisions, so that it has a team and not a workgroup is important to get this quality. It is necessary service organization, administration and leadership, commitment with the service and motivation for all their subordinates and continuous search for improvement3. Therefore, the nurse by showing up leader, becomes attentive and present to the real needs of patients, in order to avoid risks and promote holistic and humane care. To report an experience of a supervised stage of Federal University of Pernambuco academics about the importance of the service organization to the health assistance quality. It is a study of type experience report about the academics experience of nursing course of Federal University of Pernambuco, in the supervised stage in the Restauração Hospital, public hospital institution, located in the city of Recife. The stage in this entity occurred on period of June of 2015, and it was integral part of avaliative method of the curriculum component: Health system organization in differents levels of complexity. The data collection was guided through a structured screenplay, where the following items were discussed: Lifting of organizational structure of the allocated service to the practice, of human resource management and of materials flow and of sector; analysis of main points of nursing management. It was found a overcrowding of the eleven rooms that there was in the sector, where all the nine beds were occupied by patients and theirs escorts, resulting including the difficulty in the realization of nursing procedures. Performing a analysis about the management of humans resources of the Hospital and quantitative of professionals of sector, emphasizing the form of division of the nursing team, we found that there was lack of motivation for part of the team due to several factors like delay in salaries and high demand of patients to few professionals, culminating almost always in a bad quality service and including performed by professional not capable to its achievement, have also unwilling mutual help of the two wards. In addition to that, on the floor we found ninety nine patients for three daytime nurses and eleven technicians, totaling approximately forty patients for each nurse professional and nine for each one technical of nursing. In the weekend and night shift only count two nurses on the floor. It can be seen clearly the work overload to the sector professionals, making impossible an appropriate assistance to minimize the side effects related to the hospitalization patients, almost all bedridden. This overload also ends up causing disinterest and lack of time by these professionals to permanent education experience that is offered. Due to such situation, only is possible the effective realization of the first step of the patient safety, the identification. The institution in study introduces more negative results than positive ones to the analysis points, verifying that many of the difficulties go beyond the power of the nurse manager. However, some points may be improved, like propose methods to motivate the team and reduce the work overload; observe the sector’s demand to divide well the tasks of each professional, determining their role and competence; elaborate in an organized manner the team meetings and the permanent education experience to be an opportunity of listen the team placements and get them to help themselves. Like this, the nurse manager must understand his role on the team and stand as leader to facilitate and help both in the work of his team members as in health assistance offered to the patients. May be identified how is fundamental that the nursing team chief stand as leader, active in his professionals responsibilities and partner of co-workers, showing up as an example to others team members. Exercise leadership means directing the group to a common objective, the quality patient care, having a wide viewing of the workplace that is under his coordinates, to prevent difficulties and prepare the team for their responsibilities and hospital service demand. With such premises the team will be stimulated to achieve the necessary motivation for the results of their actions culminated in a quality nursing care and worthy of respect.
