Design of Fault Simulation Software for Intake and Exhaust System of Marine Diesel Engine
Aiming at the problems of high operating cost, high training cost and large site requirement of traditional marine engine platform, the software of fault diagnosis system for marine diesel engine intake and exhaust system is developed by using Visual Studio 2012 platform. The relevant mathematical model is established by numerical method, and the conversion from mathematical model to code is realized to simulate the working status of marine intake and exhaust system in normal operation and failure. The results show that the simulation system runs well with low cost and small site requirement. It can accurately simulate the normal operation and fault state of the ship intake and exhaust system. It can be used for training work and improve the actual response ability of the trainers to the fault of the ship intake and exhaust system.
Analysis to the Violation of Cooperative Principles in Daily Conversation and the Production of Humor
Humor is the flash of one’s knowledge, talent, wisdom, and inspiration in language expression as well as the ability to grasp “funny or witty imagination.”.Humorous language, like a lubricant, can effectively reduce the “coefficient of friction” between people. The principle of Cooperation was put forward by Grice, a famous American language philosopher, in a speech at Harvard University in 1967. Grice believes that in the process of people’s communication, the two sides of the dialogue seem to follow a certain principle intentionally or unintentionally in order to complete the communicative task effectively. To make the communication activities go smoothly, the communicative parties always follow the cooperative principle in the process of language communication; But sometimes, in order to achieve some kind of communicative purpose, people usually break the cooperative principles and then complete the conversation in a humorous way. This paper will analyze how daily conversation can achieve humor by violating the cooperative principle. Furthermore, it is proved that the violation of the four maxims has a strong explanatory power to the emergence of humorous language.
Logistics Service Supply Chain service quality inspection results contract coordination under information asymmetry
Aiming at the two-level service supply chain, which is dominated by logistics service provider (LS) and logistics service Platform operator (LP), it is faced with the problem of uncertain market demand and asymmetric information of logistics capacity service failure rate. Under the income distribution mode of LS and LP income sharing, this thesis uses the Stackelberg game principle to propose a cost-sharing compensation contract based on the result of capacity service quality test to coordinate the supply chain. The results show that the compensation contract of cost allocation can effectively solve the problem of information asymmetry and realize the growth of supply chain income. However, the overall supply chain income has not been maximized due to the inability to achieve the simultaneous coordination of LP’s effort level and capacity supply decision.
Emergency surgery for a Morgagni hernia causing respiratory failure
Reports of emergency surgical repair of a retrosternal hernia causing respiratory failure in an adult are rare. We treated an 82-year-old man who had been suffering breathlessness upon exertion, some speech difficulty, and, most recently, visual hallucinations. He had consulted a physician who ordered an arterial blood gas test, which revealed hypoxemia, and thoracoabdominal computed tomography (CT), which revealed a hernia that was compressing the pulmonary parenchyma. Over the next 30 days, the hernia worsened, acute respiratory failure developed, and the patient was transferred to our hospital by ambulance. The patient presented to us not only with respiratory failure but also a decreased level of consciousness. CT performed upon admission revealed prolapse of the transverse colon from the posterior surface of the sternum to the right thoracic cavity, left deviation of the mediastinum, and compression of the pulmonary parenchyma, which we believed to be the cause of the hypoxemia. With the hernial orifice appearing to be on the right, a Morgagni hernia was diagnosed. Emergency surgery was deemed necessary. An epigastric midline laparotomy incision was placed, and we observed a retrosternal hernia, with an enlarged foramen of Morgagni measuring 70mm × 50 mm as the hernial orifice. The hernia contained portions of the greater omentum and transverse colon. We returned the contents to the peritoneal cavity manually closed the hernia orifice by simple suturing and reinforced the repair with a mesh patch. With signs of cardiac failure developing, temporary noninvasive positive-pressure ventilation was instituted from postoperative day 5 to postoperative day 11, but the patient’s general condition improved thereafter, and he was discharged on postoperative day 29.
Serious Staphylococcus Aureus Infection in a Patient with Diabetes Mellitus
The immune system and metabolism are closely related and it is recognized that diabetes mellitus (DM) and its complications have a compromising impact on the degree of immunosuppression. The authors present the case of a 65-year-old man with a history of type 2 DM and ulcers of the lower limbs who went to the emergency departament with odynophagia, dysphagia, fever and cervicalgia. He had a painful cervical and supraclavicular region and highly infectious parameters in blood samples. The cervical radiography suggested vertebral fusion and cervical computed tomography with soft tissue thickening, hypopharyngeal deviation and gaseous foci suggestive of abscess. He started empiric antibiotic therapy and was performed an abscess drainage where Staphylococcus aureus oxacillin susceptible was isolated. S. aureus bacteremias have a higher prevalence in diabetics, being essential the rapid orientation in situations of bacteremia to clarify the infectious focus and immediate onset of antibiotic therapy which decreases mortality.

INCIDENCE OF GASTRITIS AND DUODENITIS IN THE POPULATION OF PERNAMBUCO: AN ANALYSIS FROM NATIONAL HEALTH INDICATORS
Introduction: Gastritis is the inflammation of the mucosa that lines the inner walls of the stomach. Duodenitis consists of an inflammation of the duodenum. The main complaints are abdominal pain and epigastric burning. One of the main risk factors for these diseases is infection with the bacterium Helicobacter pylori. In Brazil, it is estimated that 80% of the population is a carrier of the bacteria, which is alarming if compared to the world average. Objective: Analyze prevalence data of new gastritis and duodenitis cases from 2013 to 2017 per sample of municipalities in the State of Pernambuco with more than 100 new cases in the period. Methodology: It is a study in secondary databases, conducted from the Basic Data indicators of Brazil, an integral tool of the health information system, using quantitative analysis of variables. Results: The new cases in 2013 halved in 2017. The municipality of Petrolândia obtained the largest relative reduction of the new cases in the period, from 38 in 2013 to 1 new case in 2017. Recife presented 96 new cases less when comparing 2017 and 2013, which is the largest absolute reduction among municipalities. Afogados da Ingazeira, Jaboatão dos Guararapes and Pesqueira also reduced the number of new cases in the period. On the other hand, the municipality of Buíque maintained the same rate of new cases in the period. Discussion: With the exception of the municipality of Buíque, it is generally observed that there were significant reductions in the number of new cases when evaluating the totality of the municipalities of Pernambuco, which may possibly be associated with the expansion of preventive public health education policies aimed at awareness of the population about adequate food hygiene, with a view to reducing the number of infections and reducing public spending on procedures and treatments. Oral pyridostigmine for treatment of postoperative ileus associated with elevated catecholamine levels: A case report
The etiology of post-operative ileus is usually multifactorial, but increased catecholamines have been implicated as an important cause. We present a case of catecholamine-induced prolonged post-operative ileus treated successfully with Pyridostigmine. A 70-year-old male underwent a low-anterior resection and diverting loop ileostomy for rectal cancer. Immediately post-operatively he developed refractory hypertensive urgency and a small bowel ileus. Biochemical testing revealed markedly elevated 24-hour urinary metanephrines and normetanephrines. However, radiologic studies failed to identify a pheochromocytoma. The ileus persisted despite employing a multimodal regimen consisting of avoidance of narcotic pain medications, gastric decompression via a nasogastric tube, maintenance of normal levels of electrolytes, parenteral nutritional support, and early mobilization. Two weeks after the surgery the patient was treated with oral Pyridostigmine with appropriate return of bowel function. Excessive circulating catecholamines play an important role in the etiology of refractory post-operative ileus, and cholinesterase inhibitors such as Pyridostigmine could be used as an effective treatment in such cases.
Pulmonary metastasis of a Dermatofibrosarcoma of Darier and Ferrand treated by IMATINIB: a case report
Dermatofibrosarcoma (DFS) described by Darier and Ferrand represents less than 5% of soft tissue sarcomas and metastases only very rarely. We report the case of a 51-year-old patient followed for six years for a DFS of the right thigh root recurrent several times after non-optimal surgical resections. The occurrence of thoracic pain associated with a cough with dyspnea, motivated the realization of a thoracic computed tomography (CT) showing a right basal pulmonary mass with pulmonary nodules. CT biopsy of one of the pulmonary nodules confirmed the metastatic nature of these lesions. The search for translocation t (17,22) could not be performed. A treatment based on imatinib has been started. The evolution was marked by the disappearance of the respiratory symptomatology and a good radiological response. The advent of targeted therapy with imatinib transformed the prognosis for this disease, which was considered incurable at the metastatic stage.
Bovine cryptosporidiosis in calves : A Review
One of the culprits during this tough spring was a parasite called Cryptosporidium. It is an important zoonotic pathogen transmitted primarily through water.Cryptosporidiosis is a common cause of diarrhoea in young calves. It is caused by a protozoan parasite of the genus Cryptosporidium,family Crptosporididae,order Eucoccidiorida,class Coccidian and phylum Apicomplexa.The parasite infects epithelial cells in the microvillus border of the gastrointestinal tract of all class of vertebrates7.and causes severe chronic and even fatal diarrhea with malabsorption and dehydration 19 Currently,there are 16 recognized species and nearly triple this number of unnamedNeonatal calves becomes infected within the first few days after birth.Feces containing eggs are a major source of infection, but calves may also spread the parasite through direct contact. Since the egg survive well in the environment, calves can also pick up infections from water,feed and soil.In addition to calves, cryptosporidiosis affects other young animals including piglets,lamb,kid,oats,foals and fawn (farmed deer). Stressors such as inadequate milk consumption,cold weather and wind may also play an important role in determining how severe the infection will be and how long it will last.
