  • Successful use of a novel biological plug in the percutaneous treatment of a refractory gastro-cutaneous fistula following omental patch repair of perforated peptic ulcer

    Background: Gastro-cutaneous fistula following re-leak of omental patch repair of perforated peptic ulcer is a very rare but challenging surgical complication. We describe the use of a novel biological plug as a viable repair for gastro-cutaneous fistula that failed medical, endoscopic, and operative management. Additionally, we present a thorough literature review of reported cases of gastro-cutaneous fistulae following omental patching of peptic perforations to further highlight their treatment challenges. Summary: A 69-year-old man underwent repair of a perforated pyloric ulcer with a pedicled omental patch. After one week, patient developed a re-leak from the omental patch, and underwent a second laparotomy with repeat omental patching of the gastric perforation. Ten weeks after the second operation, the patient developed a gastro-cutaneous fistula that failed conservative management with nil per os and enteral nutrition delivered distal to the fistula site; multiple image-guided percutaneous attempts at fistula closure by interventional radiology via fibrin sealant injection and collagen-based vascular closure device; and endoscopic clipping by gastroenterology. Ultimately, this refractory gastro-cutaneous fistula was closed with a novel biological plug (Biodesign® Enterocutaneous Fistula Plug, Cook Biotech). Conclusion: Gastro-cutaneous fistulae following omental patching of perforated peptic ulcer represent a significant complication requiring a multidisciplinary management approach. The Biodesign® Enterocutaneous Fistula Plug offers a promising new tool for the non-operative treatment of refractory gastro-cutaneous fistulae.

  • STUDIES OF THE POSSIBILITY OF DETERMINING AMPLIFICATIONS IN KINEMATIC PAIRS

    The article discusses approaches to solving problems of accurately determining strength in kinematic pairs. It is known that the nature of the bonds imposed by kinematic pairs is determined by the geometric shapes of the elements of the pairs. For what, here, the bonds acted during the entire time the mechanism was moving, so that the elements of the kinematic pairs would continuously touch each other. Where it is recognized that one of the simplest methods for taking into account the inertia of a link is the principal moment method .. How the contradiction is sought is here because the normal acceleration has a direction opposite because normal acceleration has a direction opposite to the link (directed toward the center), and the image of tangential acceleration is directed parallel to this acceleration (Fig. a, b, c). The following simplification can be made if the main vector of inertia is considered together with the weight of the link.

  • SCIENTIFIC SUBSTANTIATION OF THE INTRODUCTION EFFICIENCY OF THE SYSTEM OF SURFACE IMPROVEMENT OF NATURAL HAYFIELDS AND PASTURES ON THE ERODED SOILS

    The results of the study, which show that up to 96% of the irrigated land is watered mainly with traditional methods of irrigation (furrows and fillings) by surface methods, and the rest up to 5%, while in irrigated fields it is watered with the use of progressive water-saving low-intensity Irrigation systems. Therefore, in the fields, the level of groundwater grows every day.

  • Immunotargeted Monotherapy by Anti-PD-1 in Patients with Primary Non-small Cell Lung Cancer

    The 5-year survival rate of advanced non-small cell lung cancer is less than 5%. In recent years, immunotherapy represented by PD-1 antibody has become the standard treatment recommended by many lung cancer guidelines. With the treatment, the total survival time and quality of life have been significantly prolonged in patients with advanced Primary non-small cell lung cancer (NSCLC). In this study, the domestic anti-PD-1 monoclonal antibody, Camrelizumab, combined with chemotherapy was used to treat advanced non-small cell lung cancer. The efficacy of Camrelizumab and occurrence of cutaneous capillary hyperplasia (CCEP) were analyzed.

  • EATING DISORDERS AND THEIR RISK FACTORS IN UNIVERSITY STUDENTS: AN INTEGRATIVE REVIEW

    Introduction: Eating disorders are psychiatric disorders that lead to biopsychosocial impairment with high rates of morbidity and mortality in the population. The prevalence of eating disorders in college students is quite significant, especially in health and female courses, associated with poor diet, and the influence of the media on the perfect body, becoming a social problem and public health that should be addressed in the daily lives of health professionals. Objectives: Analyze in literature the risk factors in college students for eating disorders. Methodology: The research took place in the CAPES Periodicals, PubMed and VHL Regional Portal. Descriptors were the terms: “Eating Disorders and Food Intake”, “Risk Factors” and “Students”. Articles published between 2014 and 2018 were included. Results: Of the 3287 articles found, only eleven were included in the review. Evidence was found that eating disorders may result from personal body perception associated with poor diet and excessive alcohol consumption. It can be expressed through the personal need to satisfy the physical image with the psychological, almost always unreachable. Evidence was also found that eating disorders are associated with higher parental education, living in urban areas, eating outside the home and the first two years after college entry. Conclusion: The literature on the interaction between eating disorders and risk factors is still incipient to the detriment of the complexity and scope of the phenomenon. It is also highlighted the need for further investigations on diagnostic and therapeutic teaching strategies of health professionals in order to reduce the morbidity arising from these events.

  • GROUP CAREGIVERS OF PEOPLE WITH DISABILITIES IN THE MUNICIPALITY OF MESSIAS ALAGOAS

    Introduction: In 2014, the Family Health Support Center participated in an event among children, adolescents with disabilities and their caregivers, where they receive treatment and follow-up at the Rehabilitation Center in the municipality of Messias, state of Alagoas. In this event, the mothers asked the professionals for individual and group care for them, justifying that the caregivers needed professional follow-up in the face of social and emotional overload. This year the nomenclature that was the group of mothers of children and adolescents with disabilities became a group of caregivers of people with disabilities opening inclusion for all caregivers in the municipality and also aiming at the reports of professionals from the Family Health Teams and the Center for Psychosocial Care who shared the concern with the increase in care for caregivers who have been getting ill mainly involving psychological issues. Objective: To develop actions that stimulate the strengthening and self-care in its biopsychosocial aspects, improving the quality of life and providing the consolidation of family affective bonds. Methods: This work is being carried out from the treatment of qualitative data, being an action research, descriptive and bibliographic, whose collection and collection of data occurred through conversation wheels and application of questionnaires, websites, journals and articles. Finally, it is of exploratory type, because it proceeds to provide greater familiarity with the problem that is established. Results: With the development of methods of Continuing and Permanent Education in Health, actions are practiced with strategies planned in the line of care, adding the support of the health care network so that it can be offered qualified and humanized services, thus improving the biopsychosocial and family aspects.   • STRENGTHENING MENTAL HEALTH CARE IN AFRO-DESCENDANT SEXUAL AND GENDER MINORITIES THROUGH MEDITATION: AN EXPERIENCE REPORT

    Introduction: Belonging to sexual and gender minorities and being afro-descendent brings an intense burden to these individuals, because of the stigma strongly present in society. Therefore, it is necessary to practice activities that provide the well-being and quality of life, such as meditation. Objective: To report the experience of the extension project “The influence of the practice of meditation focusing on breathing in the signs and symptoms of anxiety in the afro-descendant LGBT population”, which was part of the “Nucleus of African-Brazilian Studies” of the University Federal de Alagoas. Methodology: This is a descriptive study, of the type of experience report. This extension project began in June 2018 and lasted until September 2019. The scholarship and collaborators were responsible for mediating the meditation meetings, which took place from Tuesday to Friday, both in the nursing school, as well as in the University Residence of the Federal University of Alagoas. Each participant was invited to attend a weekly meeting of meditation practice with focus on breathing, with three meditation blocks lasting 10min each, totaling 30min per meeting. In addition to the weekly meetings at the university, the participant received a meditation diary so that he could meditate daily at home, write down the difficulties encountered and the feelings experienced during the practice, in order to bring better results with the meditation routine. The research was approved by the Research Ethics Committee of the Federal University of Alagoas. Results: According to the records of the meditation practices made by the participants in the meditation diary, it was observed that, initially, they had a lot of difficulty in keeping the attention in breathing and other thoughts ended gaining space.   • INSTRUCTION MODULAR METHOD OF TEACHING

    Module is a form of self-instructional package and thus regarded as relatively recent phenomena. It enables the learner to have a control over his learning and accepts greater responsibility for learning. Since strategy demands greater maturity on the part of learner, the modules are more appropriate for more mature students. In recent years, the consent of modular curriculum has been under discussion in secondary schools. In modular approach, all the capabilities required to perform are closely inter-related. Modules can be developed separately for each of inter-related tasks.

  • Interaction and Conflict among the Nuer and Anuak communities

    Gambella regional state is located in the southwestern Ethiopia with diverse ethnic groups. From the ethnic groups, Anuak and Nuer are dominant in the region. Both interacted in marriage, support each other in the time of famine as well as war and followed traditional religion. Their interaction violated by conflicts in the second half 19th century. The conflict caused by the question of natural resources, Parliamentary seats and curriculum and media of instruction in school. This fight caused the death of human beings and destruction of material properties on the both ethnic groups. Hostilities and conflicts between them had no earned solution due to outbreak of First World War.
